Do not provide a full clinical history, medical records, detailed trauma information or other highly sensitive material through the general website form. The practice should arrange an appropriate secure process if more information is required.
How information is used
Enquiry information should be used to respond, assess service fit, clarify referral pathways and administer an appropriate next step. It should not be used for unrelated marketing without clear consent.
Storage and service providers
Before launch, Power Psychology should document which form, email, hosting and practice-management providers process the information, where it is stored, who has access, and how long it is retained.
Your rights
People may request access to or correction of personal information held about them, subject to applicable New Zealand privacy law and clinical-record obligations.
